Every time I go to switch the visible patterns, in order to choose the one I want, I run into this and I never know just what I should do : What "changes" does it mean ? Many times, I haven't changed anything; do I still need to save something ? If I just click "No", what am I going to lose ? When would I want to click "Yes" and what happens after that ? Thanks for reading and all help is appreciated.

BuckSkin, I occasionally see a similar dialog box when resetting brushes after I have changed sizes or orientations and I just click No in order to preserve the original brush format.
I don't remember ever seeing it with patterns. Your screenshot looks as if you flipped the last one horizontally. Perhaps it refers to that.

BuckSkin, I have got that message when working with custom patterns. This has been my experience. As long as you stay inside the Library where your custom patterns are being displayed (as a rule of thumb -- the Default library), no message will pop up if you change from one pattern to another. On the other hand, if you decide to use a pattern from another Library, for example, Artists Surfaces, you will get that question. If you say "Yes", a window will pop up allowing you to save your pattern (.pat) in the appropriate folder within Elements.
C:\Users\[…]\AppData\Roaming\Adobe\Photoshop Elements\x.x\Presets\Patterns
If you need to use that pattern in a future project, bring up the menu and select "Load Patterns . . .".
