To summarize your screenshots: - No difference and no duplication of files when seen in the Windows explorer from card reader or from cable from camera. (I assume the four test files are not also present on the other DCIM subfolders of the card) - When you start the downloader to open the selected files in the DCIM subfolder, each file is displayed twice in your previous screenshot from the downloader. There could be a logical explanation ONLY if the four selected files were also present on another subfolder AND if the duplication filter of the downloader did not recognize files as duplicates from the usual criteria: file size in KB and date_taken. That does not make sense in normal use of a card.
Now, from your previous posts, it appears that after showing duplicates, the downloader does copy the files and duplicates in your destination folder; since Windows requires to rename duplicate files with a suffix to store them in the same folder, that would explain the renamed copies you were mentionning.
So, that excludes many suspected origins of duplicates in previous discussions and that points to a bug in the downloader to display without duplicates files from a subfolder of a card. Considering the number of PSE2021 users and the fact that the most common task for them is to download their photos to their computers, such a PSE bug should appear very frequently and be reproducible. Then, it will be possible to find if other external factors are required for the bug to appear. Maybe some of our forum friends could try themselves to reproduce the issue and report their findings here.

OK, now it's getting more interesting. I took 2 new shots with my Fuji. I downloaded two ways: via USB cable and direct insert of SD card into the computer. * The USB method resulted in duplicate images shown on the Organizer download screen. * The direct SD card insert method resulted in a SINGLE image of the 2 new shots.
Also, I went back and downloaded from my Samsung S9 Andriod phone using the USB cable method: No duplicates. (I thought I got duplicates before when I tested this, but I could be wrong.) So, what might explain getting duplicates from photos downloaded from my camera through a USB cable but not from a direct card-in-computer download? If this question is never entirely solved, at least using the "stick the SD card into the computer" method is a lot simpler than deleting 50 or 100 duplicates from the Organizer every time I download a batch using the cable.
We're making progress. Thanks for encouraging me on.

As a general rule, transferring from the camera via a cable is much slower than using a decent card reader, whether it be a card reader built into the dashboard of the computer or a separate USB-connected card reader. Speed-wise, a USB 2.0 card reader will usually out-perform transferring from the camera and USB 3.0 is even quicker. The only convincing argument of transferring straight from the camera instead of using a card reader is that, if your camera uses CF cards, with the many many tiny pins, there is always the possibility that a pin could get bent or even stick in the card and pull out of the camera; although I have never had it happen, it is not an uncommon occurrence and a good argument to leave the card in the camera and use the cable instead. It may happen to me tomorrow; but, I suspect those who have bent pins and such are of the cram-slam school and CF cards are probably not the only thing they have problems with; slow and easy, with a bit of finesse, yields a much more harmonious outcome. I never load image files straight into any program as I always encountered various problems doing so. I use D.I.M. (Digital Image Mover) to rename/number the files and copy them from the card to temporary year/month/day folders (created by D.I.M.) D.I.M. will recognize the RAW/jpeg pairs and give them the same name. After trying other naming conventions, I use 4-digit Year - 2-digit Month - Day _ HourMinuteSecond _ Camera _ sequential 4-digit number like this 2019-08-19_164239_7DMkII_0749 One would think I could do without the 4-digit sequential number; but, you would not believe how many images in a day's shooting will have identical HourMinuteSecond, especially when shooting rapid-fire.

Variables tested:- PSE 2021 installed on two computers. - Both computers running Windows 10 Pro (with the latest update). - USB 3.0 cable method. - Card reader method (both built-in and external). - Cards used -- CF and SD. Results:No duplication of files occurred.

I downloaded images with an 8-year-old Canon Powershot pocket camera I took out of mothballs - so many mothballs I had to recharge the battery:
Both direct SD insert, and USB cable methods: No duplicates.
Strangest dang thing I've seen.

I just noticed something in the direct "card in the computer slot" download. Note the "Type" column with four entries of "Adobe Photoshop Elements Image 19". Why would a photo taken in the camera refer to image "Type" as "Adobe?" Was there a "feedback loop" of some sort bouncing off the camera and reloading onto the Elements app? Or is that a normal "Type" description for something downloaded from the camera into Elements?
This is a screen shot showing the Properties of the first image...
Earlier question: Wondering why the image "Type" was listed as "Adobe Photoshop Elements Image 19" when sent from the camera in the computer's file system, totally bypassing the Elements program.
Answer: I have my defaults in the Windows 10 system set to address what it recognizes as "photos" to open via this Adobe Photoshop Elements 19 (2021)
So this has nothing to do with the download questions.
